چکیده

Achieving a high response rate for patient reported outcomes (PROs) during follow-up in RCTs is challenging. A response rate of 69% to the 12 month questionnaire in the first 20% of participants reaching the timepoint in eTHoS triggered the team to intervene. eTHoS is an ongoing surgical trial for haemorrhoidal disease with the primary outcome point at 24 months and recruitment target of 800. Responses are routinely collected by postal questionnaire or online so other methods to protect the outcome at 24 months had to be considered. The literature suggests that a small voucher payment can increase the response rate although the evidence for the effectiveness in large RCTs is fairly limited. We have designed a sub-study to investigate randomly allocating participants to receive a £5 voucher with their questionnaire at 12 and/or 24 months. In terms of the trial management there were several ethical and practical considerations. Ethical considerations included fairness in distribution of the vouchers; whether to make the voucher conditional or unconditional and being inclusive in the kind of voucher to source. When using vouchers the sponsor also has several requirements that have to be fulfilled which affect trial management including keeping track of participants that have been sent a voucher and a requirement that participants be made aware that, depending on personal circumstances, the payment may be subject to tax. The sub-study has received ethical approval and is awaiting R&D approval. Preliminary results and challenges in set-up will be presented and discussed. Tri-layer Tympanoplasty as a New Technique in High-risk Tympanic Membrane Perforations

Introduction: The eradication of the middle ear disease is mentioned as the fundamental principle of tympanoplasty. The presence of some factors related to patient or disease itself forces the physician to classify the chronic ear disease as high-risk perforations. The aim of this study was to present a tri-layer tympanoplasty technique and its otological and audiological outcomes in the ...

Forecasting Surgical Outcomes Using a Fuzzy-Based Decision System

Background and objectives: The kidneys of chronic kidney disease (CKD) patients do not have enough function and hemodialysis (HD) is a common procedure for their treatment. HD requires vascular access surgery (VAS) and arteriovenous fistula (AVF) is a low-complication method in VAS. However, different rates of AVF failure have been reported worldwide which can cause repeating s...

Strategies to improve response rates to patient reported outcome measures in a surgical RCT

Difficulties achieving high response rates to patient reported outcome measures (PROMs) within clinical trials are well recognised. The NIHR HTA funded eTHoS trial, which compares Stapled Haemorrhoidopexy with Traditional Haemorrhoidectomy for the treatment of grade II-IV haemorrhoids, is no exception. Early response rates to postal PROMs on quality of life (distributed at 12 and 24 months post...

Assessing the role of enema in improving outcomes and complications of benign gynecological laparoscopy: a randomized clinical trial

Background: mechanical bowel preparation (MBP) is a common practice before laparoscopic gynecologic surgeries but the role and efficacy of preparation have been questioned. this study assesses visualization and bowel handling in a group of patients who receive MBP and the control group and thereafter; compares the results. Methods: We designed and conducted this randomized, single-blinded and ...
